1. Register for a Boon Lay Health & Wellness Card online by clicking on this link > https://go.gov.sg/boonlay-health-card-2024. Pick up your card at Boon Lay CC by 30 June 2024.2. Participate in and complete FIVE (5) qualifying Health & Wellness activities organised in the Boon Lay Community (list below). These activities must be completed by 31st August 2024.3. Earn a $5 NTUC voucher with your completed card, by bringing it to Boon Lay Community Club by 8th September 2024. Incomplete cards will not be accepted.4. Each Boon Lay resident can only register for ONE (1) "Health & Wellness Card" during this campaign period.5.
